WHAT YOU MUST DO IN CASE OF AN ACCIDENT:

  • Please notify the Department of Public Safety.
  • Notify us at 234-5860 or visit our office within 24 hours.
  • Secure your vehicle to prevent further damage and vandalism.

CLAIMS PROCESSING:

Please note that all claims are handled by our Claims professional.  The processing of claims starts with our receipt of the complete documents . The Auto Accident Claim Form can be downloaded below.

DOCUMENTARY REQUIREMENTS:

I. Comprehensive/Collision (Own Damage)

· Accident Report, original copy

· Completed Auto Accident Claim Form

· Copy of the current Automobile registration

· Copy of the Driver’s license of the authorized driver using the automobile at the time of the accident.

· Three (3) repair estimates

II. Third Party Claim

a. Bodily Injury

· Accident Report

· Completed Auto Accident Claim Form (both by the Insured and the Third Party)

· Medical Certificates/reports

· Hospital bills and receipts, all original copy

· All other documents that may be required

b. Property Damage

· Accident Report, original copy

· Completed Auto Accident Claim Form (both by the Insured and the Third Party)

· Copy of the current Automobile registration

· Copy of the Driver’s license of the authorized driver using the automobile at the time of the accident.

· Three (3) repair estimates

WORKER’S COMPENSATION CLAIM

WHAT YOU MUST DO IN CASE OF AN ACCIDENT:

  • Please notify the CNMI Worker’s Compensation Commission.
  • Notify us at 234-5860 or visit our office within 10 days
  • File the WCC Claim Forms to the CNMI Worker’s Compensation Commission then copy to PBIC Office.
  • Call us for more information.

  • Employer’s Incident/Accident Report of the Injury
  • Copy of Illness Certification Slip from the Physician
